Registered Nurse (RN) – Community & Home Care

Pragma Home Care is seeking compassionate, reliable and experienced Registered Nurses to join our growing community nursing team across Perth Metro and the Peel region .

The successful candidates will provide high-quality, person-centred nursing care to clients in their homes, supporting their health, independence and wellbeing.

Key Responsibilities

- Conduct nursing assessments and monitor clients’ health and wellbeing.
- Medication administration and management.
- Wound care and dressing changes.
- Catheter, continence, diabetes and chronic disease support.
- Monitor vital signs and recognise changes in clients’ conditions.
- Escalate clinical concerns appropriately.
- Provide health education and support to clients and families.
- Maintain accurate and timely clinical documentation.
- Liaise with GPs,



allied health professionals and other healthcare providers.
- Follow infection control, clinical governance and workplace safety requirements.
- Current AHPRA registration as a Registered Nurse .
- Current National Police Clearance.
- Current First Aid and CPR.
- Current Australian Driver’s Licence and reliable vehicle.
- Solid clinical assessment, communication and documentation skills.
- Ability and willingness to provide community-based services across Perth Metro and Peel areas .
- Community nursing, home care, aged care or hospital experience is highly regarded.
