Are you passionate about helping people access the right care and support? We are seeking a compassionate and highly organised My Care Clinic Intake Officer / Service Navigator to join our team.

In this important frontline role, you will be responsible for the administrative coordination, service triage, navigation and ongoing management of referrals received through MYCARE Specialist Clinics and associated service pathways.

Working with a high degree of autonomy, the position is responsible for managing complex referrals, identifying barriers to care,



coordinating multidisciplinary service access and supporting achievement of specialist clinic access performance targets.
