We are looking for a compassionate, qualified, and experienced Registered Nurse (RN) to join our team and deliver high-quality community nursing services to eligible clients.

Position Details

· Position: Registered Nurse – Community Nursing

· Organization: Activate Care

· Location: Greater Geelong, North, East, South & West Geelong

· Employment Type: Casual / Part-Time

Key Responsibilities

· Provide community nursing services to eligible clients in their homes and community settings.

· Conduct nursing assessments and monitor clients’ health conditions.

· Develop, implement, and review individualized nursing care plans.

· Deliver wound care, medication support, catheter care, and other nursing interventions within your scope of practice and authorized service arrangements.

· Maintain accurate clinical documentation and nursing records.

· Liaise with general practitioners, allied health professionals, families, and other relevant stakeholders.

· Ensure compliance with professional nursing standards, clinical guidelines, and organizational policies.

· Promote clients’ dignity, independence, safety, and overall well-being.

Essential Requirements





· Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.

· Relevant nursing qualifications and clinical experience.

· Strong clinical assessment, communication, and documentation skills.

· Ability to work independently and manage community visits effectively.

· Current First Aid and CPR certification, as required.

Desirable Experience

· Experience in community nursing, home-based care, or wound management.

· Experience supporting older people, people with complex health needs.
