Subcontract Support Coordinator – Girasole Support

Girasole Support is seeking an experienced, compassionate Support Coordinator to join our team on a subcontract basis.

We support NDIS participants to understand their plans, access suitable services and work towards their goals. We’re looking for someone who builds strong relationships, takes initiative and follows through on commitments.

About the role

- Provide person-centred Support Coordination to NDIS participants.
- Help participants understand their funding and make informed choices about their supports.
- Connect participants with providers and coordinate communication across their support teams.
- Monitor budgets, service agreements and progress towards goals.
- Prepare reports and gather evidence for plan reviews and requests.
- Maintain accurate, timely case notes and records of work completed.
- Identify and respond to service gaps, barriers and emerging concerns.

About you





- Experience delivering NDIS Support Coordination and a sound understanding of the NDIS.
- Solid communication, organisation and problem-solving skills.
- Confidence working independently and managing competing priorities.
- A commitment to participant choice, control and respectful practice.
- Relevant qualifications in community services, disability, social work or a related field.
- An ABN, appropriate insurance and relevant worker screening checks.

Pay: $55.00 – $65.00 per hour
