At People First Healthcare, we support – Support at Home participants to remain independent, safe, and well in their own homes. We are seeking a Virtual Registered Nurse to provide clinical oversight, assessments, and coordination of care for our community-based clients.

About the Role
n
This is a predominantly virtual role, supporting Support at Home participants through telehealth, care planning, and clinical coordination, with minimal travel required.
n
Key responsibilities include:
n
n
- Conducting virtual clinical assessments and reviews
n
- Developing and reviewing care plans for Support at Home participants
n
- Providing clinical advice and guidance to care workers
n
- Monitoring participant health outcomes and escalation of care when required
n
- Liaising with participants, families, GPs, and allied health professionals
n
- Ensuring compliance with aged care standards and clinical governance requirements
n
- Accurate clinical documentation and reporting
n
n
About You
n
n
- Current AHPRA registration as a Registered Nurse
n
- Experience in aged care, community nursing, or Support at Home
n
- Confidence delivering care and assessments via telehealth / virtual platforms
n
- Strong clinical judgement and communication skills
n
- High level of organisation and autonomy
n
- Sound knowledge of aged care quality standards
n
