Purpose of Position
n The Personal Care Worker assists in the provision of holistic client focused and consumer directed care in accordance with an
n established care plan. The Personal Care Worker provides for the personal needs and comfort of clients in their own home.

Essential Functions n

n
- Adheres to the organisations policies and procedures.
n
- Performs personal care activities contained in the written client care plan to assist the client with their activities of daily living. The
n Personal Care Worker performs the assigned tasks as instructed by the Registered Nurse in the Client Care Plan. These activities
n include but are not limited to personal hygiene, toileting, nutrition, and mobility.
n
- Actively participates in identifying client care needs, implementing and evaluating the needs of the client in their Home in
n conjunction with the Registered Nurse.
n
- Cares for client in an organised manner in their own home, prioritising their care needs
n
- Enhances the integrity of clients by respecting their personal values, beliefs, and customs and protecting their rights.
n
- Collaborates with other members of the client’s care team in the delivery of effective, holistic and client focused care.
n
- Communicates effectively with client’s and significant others, and other team members using a variety of techniques (written, oral,
n non-verbal), which ensures confidentiality of client information.
n
- Actively follows the organisations policies, procedures and mission and uses a range of competencies and knowledge that are
n applicable to the role of the Personal Care Worker. These include various nursing actions (which are taught by a RN for a specific
n client such as:n n




- Assisting in changing ostomy appliances
n
- Reinforcement of non-sterile dressings
n
- Assisting with mobility aids (walking stick, rollator, 4-wheel walkers, wheel chairs, hoists, walk belts and slide sheets)
n
- Assisting with prescribed Range of Motion exercises
n
- Measuring and preparing diets
n
- Measuring intake and output
n
- Taking vital signs to report to RN with automated device (may not interpret)
n
- Application of urodomes (External urine collection bags)
n
- Assist with the application of slings and compression stockings
n

n
- Participates in mandatory education, In-services, and Continuous Quality Improvement program, which provide a focus on client
n satisfaction with the care and provision of a safe setting for the client in their own home. Adhering to Work Health and Safety
n and Infection Control policies.
n
- Participates in own performance appraisal to self-assess and identify goals for further development and achievement.
n
- Maintains up-to-date knowledge and skills by pursuing continuous professional development according to individual identified
n needs.
n

The Personal Care Worker performs light housekeeping tasks that will facilitate the client’s self-care at home. These tasks include

n

- Vacuuming
n
- Dusting
n
- Sweeping and mopping floors
n
- Doing dishes
n
- Laundry
n
- Cleaning bathrooms
n
- The Personal Care Worker is responsible for the documentation of service provided, using the Visual Care app and the client care record.
n
- The Personal Care Worker will refer all questions or reports of client concerns or changes in condition immediately to the
n Registered Nurse On-Call. Personal Care Workers are not to contact medical practitioners directly.
