EIH Care is seeking a compassionate, skilled and reliable Registered Nurse (Community based) to join our team, supporting clients across our NDIS and Aged Care services.
This is an excellent opportunity for a nursing professional who enjoys working autonomously, delivering high-quality person-centred care, and making a positive impact on the health, wellbeing and independence of clients in their own homes.
About the Role
As an Registered Nurse, you will provide safe nursing care to clients with a range of clinical and support needs. You will work collaboratively with clients, families, support workers and other healthcare professionals to ensure care is delivered in line with individual care plans, clinical best practice and relevant regulatory requirements.
The role offers flexibility and variety, making it well suited to a nurse seeking casual shifts and the opportunity to contribute to a supportive, client-focused care environment.
Key Responsibilities
Your responsibilities will include, but are not limited to:
- Clinical Assessments: Conduct falls risk, continence and dementia assessments, identifying risks and implementing appropriate care strategies.
- Personal Care: Provide clinical support and assistance with personal care needs while promoting dignity, comfort and independence.
- Continence and Urinary Management: Support clients with urinary and continence needs, including assessment, care planning and ongoing management.
- Catheter Care: Provide catheter management and associated clinical care in accordance with best practice guidelines.
- Diabetes Management: Support clients with diabetes management, monitoring, education and implementation of individualised care plans.
- Wound Care Management: Assess, treat and monitor wounds, document progress and escalate concerns when required.
- Medication Management: Support safe medication practices, administration where authorised, monitoring and documentation in accordance with relevant policies and procedures.
- Clinical Documentation: Maintain accurate, timely and confidential clinical records, assessments and progress notes.
- Care Coordination: Communicate effectively with the wider care team and relevant health professionals to support continuity of care.
- Clinical Escalation: Recognise changes in a client's condition, respond appropriately and escalate clinical concerns in a timely manner.
About You
We are looking for a nurse who is professional, adaptable and passionate about delivering high-quality community-based care.
To be successful in this role, you will have:
- Current registration as a Registered Nurse through AHPRA.
- Experience in community nursing, aged care or disability support
- Demonstrated clinical skills across wound care, continence management, catheter care, diabetes management and medication management.
- Solid assessment, clinical reasoning and decision-making skills.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families and colleagues.
- Valid Australian work rights are required.
- Current NDIS Worker Screening Check, Working with Children Check and Police Check.
- A current driver's licence and reliable transport to travel to clients.
