- $80,008 to $86,588 (pro-rata) per annum, plus superannuation

About the role

You will be responsible for managing a case load of Support at Home Participants. This involves working closely with each Participant and their families to ensure they receive an individualised, responsive service under a consumer directed care model.



You will also be responsible for all client documentation and the maintenance of individual client budgets

Skills and Experience

We're seeking someone with a Minimum Certificate IV qualification. A degree or diploma in nursing or a health-related discipline preferable.
