Allegiance Heart & Home Care is seeking a reliable and compassionate Community Support Worker to support aged care clients across Sydney and surrounding suburbs.

The successful applicant will provide person-centred support that promotes independence, dignity, choice and wellbeing in the client’s own home and community.

Key Duties
Personal care, including assistance with showering, dressing, grooming and other daily living activities
Social and emotional support
Companionship and meaningful engagement
Assistance with community access and appointments where required
Domestic assistance, including cleaning, laundry, changing bed linen and general household tasks
Meal preparation where required
Supporting clients in accordance with their individual care plans
Observing and reporting any changes in the client’s health or wellbeing
Completing progress notes after each shift
Following all workplace, infection control and safety procedures

Comfortable working independently in clients’ homes
Able to provide personal care confidently and respectfully
Able to provide appropriate social and emotional support
Able to follow care plans and instructions
Skilled when communicating with clients, families and the care team
Committed to maintaining privacy, dignity and confidentiality

Requirements

Applicants should have:
Certificate III in Individual Support, Aged Care or equivalent
Current National Police Check
Current First Aid and CPR
Valid Australian work rights
Reliable transport
Ability to complete electronic progress notes
Previous aged care, home care or community support experience highly regarded
