BlueCare seeks a Clinical Care Partner to oversee and coordinate complex clinical and care needs for community clients. You will assess, plan, implement, and monitor care with a multidisciplinary team, spending most days in the community to ensure person-centred care.
Role requires current AHPRA registration, a valid driver’s licence, and access to a suitable vehicle. The position is full time, with advantages under UnitingCare’s family of services in Queensland.
