Salary: $80,000 per annum + superannuation
AQF Diploma or higher qualification and relevant experience.
At least one year of experience supporting families, carers or vulnerable clients in a community, aged care, disability or family services setting.
Knowledge of Australian aged care and disability systems, including Support at Home, My Aged Care and the NDIS.
Experience assessing support needs, setting goals with families and maintaining accurate case records.
Robust communication, active listening and conflict-resolution skills, with the ability to work sensitively with people from diverse backgrounds.
Ability to work collaboratively with clients, families, health professionals and community service providers.
Current NDS Worker Screening Check and National Police Check.
Current NSW driver licence and access to a reliable vehicle for client and family visits.
