Ballarat Community Health is seeking a Care Navigator for its Seniors Lifestyle Victoria program in Ballarat. You will provide person-centred care coordination to help older adults live independently at home, guiding participants through tailored plans to build capacity and wellbeing.

In a cooperative multidisciplinary team, you will work with allied health, nursing and general practitioners to deliver holistic support, manage complex needs and promote social connection.
