Mission Australia is seeking a proactive Carer Support Liaison Officer to join our Carer Gateway team in Nowra.
You will coordinate planned and emergency supports, manage provider relationships, and ensure compliance across documentation and brokerage processes.
You'll work with Carer Support Planners and external service providers to connect carers with in-home or facility-based supports, while maintaining accurate records and client information.
