The exciting opportunity:
- Community-based role supporting older people across Perth metro
- Independently manage a caseload of Home Care Package and Support at Home clients, overseeing their care needs, services and individual budgets
- Conduct comprehensive assessments, develop and review care plans, and coordinate services as client needs change
- Monitor package expenditure and funding arrangements to ensure services remain appropriate and within available budgets
- Work closely with clients, families, support workers and healthcare professionals to coordinate safe, responsive and person-centred care
- Provide clinical oversight and support, identifying changes in client health and wellbeing and responding appropriately
- Maintain accurate documentation, manage client reviews and contribute to quality and continuous improvement

About you:
- Current AHPRA registration as an Enrolled Nurse or Registered Nurse
- Demonstrated experience independently managing Home Care Packages or Support at Home client caseloads
- Solid understanding of individual package budgets, care planning, assessments, reviews and service coordination
- Sound clinical judgement and confidence identifying and responding to changing client needs
- Good knowledge of aged care legislation, quality standards and contemporary home care practices
- Excellent communication, organisational and relationship-building skills, with the ability to manage competing priorities
- Confident working autonomously and taking responsibility for your caseload, while collaborating effectively with colleagues and external providers
