Disability Support Worker – Supported Independent Living (SIL) – West Pennant Hills & Merrylands

Quality Health Care (QHC) is looking for experienced Disability Support Workers for two separate Supported Independent Living (SIL) opportunities, located in West Pennant Hills and Merrylands.

Both roles involve providing 24/7 support to residents living with Huntington's Disease, mental health conditions, and complex communication needs, including non-verbal communication. Residents require a high level of physical support and assistance with daily living activities. As a Disability Support Worker, you will play a key role in promoting dignity, independence, and quality of life while delivering safe, person-centred support within the home and community.

Your Role:

- Supporting residents with personal care, continence management and mealtime assistance

- Providing manual handling support using appropriate equipment and techniques

- Medication administration and accurate documentation

- Supporting community access, appointments and participation in meaningful activities

- Encouraging independence and maintaining a safe, supportive living environment

- Implementing positive behaviour support strategies in line with individual support plans

- Monitoring changes in resident wellbeing and reporting concerns appropriately

- Completing progress notes, incident reports and other documentation as required

What We're Looking For:

- Cert IV in Disability, Individual Support,



Community Services or equivalent experience

- Experience supporting people with high physical support needs, neurological conditions, secondary health conditions or behaviours of concern

- Solid skills in manual handling, personal care and mealtime support

- Understanding of Positive Behaviour Support and restrictive practices

- Strong communication, documentation and interpersonal skills

- Medication administration experience highly regarded

- Open availability across weekdays, weekends and overnight shifts

- Valid Driver's Licence

- NDIS Worker Screening Check, Working with Children Check, and current First Aid & CPR
