Job Description

Ballarat Community Health is seeking a Care Navigator for the Seniors Lifestyle Victoria Program. You will provide person-centered care coordination, guiding participants through tailored plans to support independent living at home.

You will work with a multidisciplinary team to reduce frailty, minimise hospitalisations and help clients achieve meaningful, self-defined goals. Applicants should have robust case management capabilities in community aged care and experience supporting older adults
