Job Description

Churches of Christ is seeking a dedicated Support Practitioner to join our Children, Youth & Families team in the Moreton Bay / Caboolture area. You will conduct comprehensive assessments, develop tailored care plans, provide crisis intervention, and advocate for clients.

Role requires collaboration with agencies, delivery of parenting programs, robust documentation, and cultural safety in service delivery. National Police Check and Blue Card are part of essential requirements.
