Melbourne Health is seeking a Progression of Care Navigator to support timely patient care and discharge planning at the Royal Park Campus, Rehabilitation Ward. This part time role coordinates routine and complex discharge plans with patients, families and the clinical team to improve ward access and patient flow.
You will be registered as a Division 1 RN or Allied Health Professional with AHPRA and have at least five years’ sub-acute experience and strong communication with diverse stakeholders.
