As part of a growing Homecare service model, this newly created dual Registered Nurse / Customer Care Coordinator role contributes directly to improving customer outcomes, strengthening care quality, and supporting sustainable workforce delivery across the community. You’ll be joining a small, collaborative team environment with strong autonomy and real impact, based in Osborne Park.

Designed to strengthen clinical oversight and customer care delivery, this position ensures we maintain high-quality, in-house clinical governance whilst also aligning with a broader commitment to delivering high-quality healthcare services and customer outcomes. Day to day you will find yourself:

- Managing a portfolio of Home Care clients
n
- Reviewing care plans and providing clinical advice on behalf of the team
n
- Reviewing and maintaining care plans, and service delivery outcomes
n
- Supporting funding, service agreements, and ongoing compliance requirements
n
- Working closely with internal teams to ensure seamless service delivery
n
- Maintaining strong relationships with clients,



families, and stakeholders
n
- Supporting continuous improvement and customer experience initiatives
n
- Ensuring documentation, case notes, and reporting are accurate and up to date
n

You’re a confident, people-focused RN who brings both clinical credibility and warmth in customer engagement. You will likely have:

n

n
- RN qualification with experience in home care, aged care, or community nursing
n
- Exposure to residential or community-based care environments
n
- Strong care planning and clinical assessment capability
n
- Highly organised with excellent communication and rapport-building skills
n
- A natural ability to build trust quickly with clients and families
n
- Experience or interest in moving away from facility-based work into community care.
