Moorditj Koort Aboriginal Corporation in Western Australia seeks a Care Coordinator – Integrated Team Care (ITC) to support Aboriginal and Torres Strait Islander clients with chronic and complex health conditions in accessing coordinated, culturally safe care.

You will work with GPs, allied health, hospitals, and community services to develop and monitor client care plans, advocate for culturally secure care, provide health education, and maintain detailed documentation.
