Care Connector (Holistic Care Solutions) is seeking Enrolled Nurses and experienced Disability Support Workers for opportunities across the Northern Gold Coast and Logan regions. This role involves delivering care in participants’ homes and community settings with ongoing clinical support.

You will work with consistent participants, support complex and high-intensity needs, and be supported by experienced clinicians.
