About the Role n

n
- n

Assist participants to access their community and attend appointments

n
- n

Deliver clinical and personal care: manual handling, hoisting, continence, PEG feeding, medication administration

n
- n

Support daily living: hygiene, meals, mobility, transport

n
- n

Monitor health changes and follow clinical care plans

n
- n

Maintain accurate case notes and incident reports

n
- n

Collaborate with families, allied health and nursing teams

n
- n

Ensure a safe, clean, and engaging home environment

n

About You n

n
- n

Minimum 3-6 months’ experience or Min Certificate III/IV in Disability, Individual Support, Nursing, or related fields.

n
- n

Experience with high physical needs and clinical care

n
- n

Confident in manual handling, medication administration, and hoist transfers

n
- n

NDIS Worker Screening, First Aid, valid Driver’s Licence, and insured vehicle

n
