Job Description

Ballarat Community Health is seeking a Care Navigator for its Seniors Lifestyle Victoria Program in Ballarat. The role focuses on person-centred care coordination to help older adults live independently at home, with support plans tailored to individual needs.

You will work within a multidisciplinary team, guiding participants toward goals, reducing hospitalisations and enhancing wellbeing. Proficiency with PRODA, My Aged Care and MS Office is desirable.
