Job Description

Metro South Health is seeking a Senior Social Worker to provide expert social work services, discharge planning, and complex case management for long-stay patients across QEII Jubilee Hospital and Metro South facilities.

You will work with multidisciplinary teams to overcome barriers to discharge, offering clinical guidance in disability, guardianship and psychosocial assessment and contributing to service development and education.
