Job Description

St Vincent’s Health Australia is seeking a Senior Social Worker (0.7 FTE, 12-Month Fixed Term) for Fitzroy. You will lead a multidisciplinary social work team, deliver psychosocial assessments, discharge planning and safeguarding, and build capacity within the service.

You will work closely with internal and external partners, supervise students and early career social workers, and contribute to improving inpatient discharge pathways as part of a comprehensive, person-centred care approach.
