Our Stafford Home and Community Service is looking for a dedicated and community focused Physiotherapist to join our Allied Health team. Pay Rates starts from $44.65 - $59.07/hr depends upon qualification and experience.

Responsibilities of the role include the following:
n
n
- Manage internal and external referrals for physiotherapy services.
n
- Complete client assessments on admission to physiotherapy.
n
- Determine appropriate goals and develop the cooperative plan of care to meet these goals.
n
- Select and provide appropriate treatment and advice to clients and carers.
n
- Obtain appropriate equipment as required by liaising with appropriate agencies.
n
- Refer to other services when required to meet the client's needs.
n
- Encourage rehabilitation and reablement by assisting the client and carers towards self-care and maximum independence.
n
- Work effectively in multidisciplinary teams to maximise client outcomes, including attending case conferences as required to advise on client care, assessment results and progress.
n
- Ensure all physiotherapy services reflect best practice, including evidence based practice.
n

Mandatory requirements:
n
n
- Current National Police Certificate OR NDIS Worker Screening Clearance
n
- Current Drivers Licence with own reliable vehicle
n
- Degree in Physiotherapy
n
- Current registration with AHPRA
n
- First Aid and CPR Certification
n
n

- Demonstrated clinical knowledge and ability to competently assess and treat aged care clients in a community setting.
n
- Demonstrated ability to develop,
implement and review Physiotherapy services.
n
- Demonstrated ability to work collaboratively with clients and their families and/or carers to maximise client outcomes.
n
- Demonstrated ability to work within a community based multidisciplinary team environment, including the capacity to negotiate with a range of stakeholders, operate flexibly, and have the capacity to problem solve.
