Job Description

Diabetes Australia in Brisbane—Milton, QLD—seeks a Telephone Health Coach for the My Health for Life program. You will guide participants through a six-session, six-month coaching plan by phone, using motivational interviewing to foster lasting health changes.

The role is part of Queensland's largest preventive health initiative, with state-wide impact. You will manage a dedicated caseload, collaborate with a supportive team, and help meet program targets while delivering person-centered care.
