n Our client is a community nursing and support service provider who has been servicing Western NSW for over 20 years. They are looking for a passionate Client Service (Care) Manager with a genuine interest in Aged Care to provide service to their clients living in their own homes in Dubbo and the Central West NSW.n
As a Client Service (Care) Manager, you will be responsible for: n

- Delivering person-centered co-ordination to clients

- Attending consultation visits.

n
n
- Completing initial intake assessments.

n
n
- Developing care plans in line with their budget.

n
n
- Case management and monitoring and reviewing client well-being

n
n

n

- Current drivers’ license and reliable vehicle

- Excellent communication skills
