Key Responsibilities n

Conduct home visits and build trusted relationships with members and families

n

Complete care assessments, care plans, and ongoing reviews

n

Identify changes in health, mobility, or support needs

n

Coordinate services including domestic support, personal care, allied health, and social support

n

Monitor member funding and ensure services align with package budgets

n

The Care Manager will manage a small local team including a Care Coordinator and Rostering & Recruitment Coordinator, supporting them to deliver high-quality services to members

n

Maintain accurate documentation and ensure compliance with aged care quality standards

n

Support service quality and continuity of care for members

n

Assist with the early growth of Maree Care's presence in Adelaide





Who We're Looking For n

You're someone who combines clinical understanding with strong relationship skills.

n

You may currently be working as a:

n

Care Manager

n

Care Coordinator ready to step up

n

Clinical Lead within an aged care provider

n

You bring:

n

Experience working within Home Care Packages / Support at Home

n

Strong understanding of aged care services and funding structures

n

Confidence conducting home visits and care planning

n

Ability to build robust relationships with members and families

n

Excellent organisation and documentation skills

n

Experience in community aged care is essential.

n
