About the opportunity
The organisation supports older Australians to remain independent and receive quality care in their own homes.
As a Care Coordinator, you'll help clients and families navigate their home care services, understand their budgets and ensure their care arrangements run smoothly.
Unlike traditional fixed-caseload models, this team takes a shared approach to managing enquiries and service requests. You'll have the autonomy to take ownership of matters as they arise, working alongside other coordinators, support workers and clinical professionals to see them through to resolution.
It's a role for someone who enjoys variety, can think on their feet and takes pride in delivering a great client experience.
What you'll be doing
- Building trusted relationships with clients and families, responding to enquiries and coordinating services.
- Helping clients navigate their Support at Home budgets, care arrangements and available services.
- Coordinating support workers, schedules and service delivery as client needs change.
- Resolving concerns, escalating clinical matters appropriately and maintaining accurate client records.
- Supporting new client onboarding and contributing to a positive, responsive client experience.
About you
You understand the realities of care coordination. You know that no two days are the same, priorities can change quickly and good communication makes all the difference.
You'll bring:
- Experience in aged care, home care or care coordination, with a sound understanding of client services and care delivery.
- Robust relationship-building and communication skills, with the confidence to manage challenging conversations.
- Initiative, sound judgement and the ability to prioritise independently and follow through on commitments.
- Basic computer skills and the willingness to learn new client management systems.
A current driver's licence, NDIS Worker Screening Clearance and First Aid/CPR certification.
Relevant qualifications are welcomed, although practical experience and demonstrated capability are particularly important.
