My Guardian is seeking motivated Registered and Enrolled Nurses to join us as Clinical Care Coordinators, growing our home‐based aged and disability care services across NSW.
As a Clinical Care Coordinator you will lead care planning, mentor staff, provide hands‐on clinical care, coordinate continuity of care between clients, families, hospitals and allied health, and advocate for NDIA and My Aged Care compliance.
