About the Role Lead the multidisciplinary team at CALHN's Urgent Care Hub – BRIGHT Sefton Park.
Shape safe care in the community, support patients with complex health needs and guide evolving models of care that help people avoid hospital where appropriate.
• Lead nursing and allied health staff in community care • Manage service demand, staffing and workload allocation • Build partnerships and coordinate care pathways • Guide service change and develop team performance • Monitor safety, quality, service data and patient outcomes About You To be successful in this role you will:
