About us

For more than 30 years, Rural Lifestyle Options Australia has been committed to bridging gaps in support across rural and regional communities.

Originally founded to provide high-quality disability support, our organisation has since expanded to deliver a broad range of services under the NDIS, as well as personalised Aged Care Support at Home (SAH) that empower older Australians to live independently at home for longer.

As an organisation, we are passionate about supporting others to chase their dreams and aspirations. Reinforced by our purpose of ‘Tailored supports and compassionate care delivered by our valued team to inspire individuals to lead independent and fulfilling lives within the community’ we achieve this – one person and one life goal at a time.

Rural Lifestyle Options Australia’s commitment to the people who access our services remains resolute, proudly underpinned by our organisation’s motto of “Big enough to provide the service, small enough to look after you.” For more information about who we are and what we do, please visit our website at www.rloa.org.au

About the role

As a Care Partner, you'll play an important role in helping older Australians navigate their Support at Home journey.

You'll be a trusted point of contact for clients and their families, taking the time to understand what matters most to them and ensuring their services continue to reflect their individual needs, preferences and goals.

From welcoming recent clients and developing personalised care plans to reviewing services and coordinating ongoing supports, you'll help make a real difference to their everyday lives.

Working closely with our Care Manager, Workforce Coordinators and broader team, you'll help ensure clients receive safe, responsive and person-centred care.

What you'll be doing:

- Build genuine, trusted relationships with older Australians, their families and representatives.
- Coordinate client intake, onboarding and commencement of services.
- Undertake client assessments and develop personalised, goal-focused care plans.
- Conduct regular care reviews, wellbeing checks and home visits.
- Monitor client needs, service arrangements and funding to ensure supports remain appropriate.
- Work collaboratively with Workforce Coordinators to support continuity of care.
- Liaise with health professionals, community providers and other stakeholders to achieve positive client outcomes.

- Identify changing needs, respond to concerns and advocate for client choice, dignity and independence.

You'll manage an allocated portfolio of clients, with consideration given to individual support needs and complexity.

We will provide you with access to a fully maintained, home garaged, RLOA vehicle (and Fuel Card) to attend appointments and events as required.

We are a small and supportive team who get to see our vision of ‘Changing lives, stronger communities’ come to life through the work that we do.

About you

You're someone who genuinely cares about people and takes pride in delivering a positive client experience.

- A Diploma or higher qualification in Community Services, Individual Support, Ageing Support, Human Services or a related field, or equivalent demonstrated experience.
- Experience undertaking client assessments, developing care plans and coordinating services.
- A sound understanding of the Support at Home program and aged care requirements.
- Confidence in managing client services, changing support needs and funding arrangements.
- Strong communication, relationship-building and organisational skills.
- Good judgement, attention to detail and confidence using client management systems.

Experience supporting older people living with dementia or complex needs, or working in regional communities, will be highly regarded.
