Primary Care Connect in Shepparton is seeking a Supported Accommodation Women's Worker to assist women transitioning from AOD programs into housing for 3 to 12 months.
You will support women and their children to access long-term housing, attend housing provider appointments, and develop life skills.
Reporting to the Harm Reduction Manager, the role involves completing assessments and care plans and coordinating with external services.
