Primary Care Connect is seeking a Registered Nurse to join our Primary Health Team in Shepparton on a part time fixed-term basis until 30 June 2027. The role focuses on chronic disease education and early intervention, delivering direct care to clients with COPD, cardiovascular disease and diabetes, and assisting with self-management strategies.
You will work with the broader PCC team, providing range of primary care initiatives and direct client services, in line with best practice standards
