Bendigo and District Aboriginal Cooperative (BDAC) is seeking an Intake, Assessment Planning Case Manager to provide culturally secure intake, assessment and short-term support for Aboriginal and Torres Strait Islander Community members in need.
The role focuses on timely access to services, coordinated care and improved wellbeing outcomes.
You will undertake screening, develop plan-based supports, and coordinate referrals while advocating for housing, health, and related services.
