?? Salary: $85,000 – $110,000 + super ? Salary Packaging: Up to $15,900 tax-free + additional $2,650 per year (meals & entertainment) About the Role

We are seeking an experienced Registered Nurse to join our Community Services Team , supporting older people to live safely and independently at home under the Support at Home Program . This role combines clinical care, care coordination, education, and advocacy , working across home care services, group social support programs and seniors’ activities. You will deliver person-centred care with a strong focus on wellness, reablement, and independence .

Key Responsibilities

- Conduct comprehensive client assessments and develop care plans
- Coordinate in-home services and liaise with external providers
- Provide education, guidance, and advocacy for clients and families




- Monitor care budgets and ensure compliance with aged care standards
- Build strong, trusted relationships with clients and stakeholders

About You

- Current AHPRA Registered Nurse registration
- Experience in aged care, community nursing, or care management
- Sound knowledge of the Support at Home Program and aged care standards
- Strong communication, organisation, and time-management skills

Current NSW Driver’s Licence with a reliable, insured vehicle

Requirements

- Right to work in Australia
- Valid AHPRA registration
- Pre-employment medical, police check, and required vaccinations
