HealthcareLink Trust in Sydney is seeking an experienced Registered Nurse to join our Community Services team as a Clinical Care Partner. You will deliver person-centred care in the home, coordinating services and supporting wellness and independence under the Support at Home Program.

This role requires AHPRA registration and experience in aged care or community nursing, with solid communication and organisational skills and a valid NSW driver’s licence.
