Aboriginal and Torres Strait Islander Hospital Liaison Officer

Location: Aboriginal and Torres Strait Islander Liaison Service – Cairns

Remuneration: $48.99 - $67.37 per hour

- Assess the needs of Aboriginal and Torres Strait Islander patients and their families/carers and provide relevant culturally appropriate intervention, i.e. problem solving and practical assistance.
- Liaise between Aboriginal and Torres Strait Islander patients/families/carers, staff and other agencies to assist with effective communication.
- Advocate for Aboriginal and Torres Strait Islander people in relation to their needs while patients of Cairns and Hinterland Hospital and Health Service to facilitate culturally appropriate health care and improve health outcomes for patients and their families.
- Provide comprehensive patient management within a multi disciplinary team by providing information about hospital services, participation in discharge planning, attending family meetings etc.
- Maintain up-to-date knowledge and liaise with relevant community agencies, support networks and accommodation services. Identify gaps in services and/or resources and where appropriate assist with service/resource development.
