Salary: $80,000 per annum + superannuation

- AQF Diploma or higher qualification and relevant experience.
- At least one year of experience supporting families, carers or vulnerable clients in a community, aged care, disability or family services setting.
- Knowledge of Australian aged care and disability systems, including Support at Home, My Aged Care and the NDIS.
- Experience assessing support needs, setting goals with families and maintaining accurate case records.
- Robust communication, active listening and conflict-resolution skills, with the ability to work sensitively with people from diverse backgrounds.
- Ability to work collaboratively with clients, families, health professionals and community service providers.
- Current NDS Worker Screening Check and National Police Check.
- Current NSW driver licence and access to a reliable vehicle for client and family visits.
