Position summary The Care Partner is each participant’s primary point of contact: coordinating assessment and care planning, managing the participant’s budget and services within their classification, and making sure services match the participant’s goals, preferences and Support at Home Service Agreement.

Key responsibilities

- Manage a participant caseload across classifications 1-8 and short-term pathways, understanding each participant’s case mix and any special needs groups.
- Coordinate initial assessment, Care and Service Plan & Budget development and reviews (at least annually and after any significant change), with the participant and their supporters leading decisions.
- Track and manage participant budgets in Visual Care; schedule only services and purchases within the Support at Home Service List; monitor spending against the plan and monthly statements.
- Record participant movements, case notes,



reviews and incidents in Visual Care the day they occur; report service information monthly to the Finance Manager and CEO for the Executive report.
- Acknowledge complaints within 24 hours and support participants to use advocacy (OPAN) and interpreters at no cost.
- Attend management and clinical governance meetings; escalate risks, deterioration and safeguarding concerns immediately to the Registered Nurse or Clinical Care Manager.

Qualifications and requirements

- Experience in aged care, community services or care coordination; Certificate III/IV in Individual Support, Community Services or equivalent desirable; robust plain-language communication with older people and families.
- Pre-employment and ongoing requirements: NDIS Worker Screening Check (recognised for aged care) or National Police Certificate; reference checks; right-to-work evidence; sign the Staff Code of Conduct and comply with the Code of Conduct for Aged Care; complete induction and mandatory training.
