NSW Health at Westmead Hospital seeks an experienced Social Worker Level 3 to manage advanced social work assessments and discharge planning for older adults with complex mental health needs. You will lead integration with families, carers and community services in a recovery-focused setting.

Join a supportive team at the Older Persons Mental Health Unit, contributing to protected discharges, care coordination and service improvement. Tertiary qualifications and AASW eligibility are required.
