Silverchain is seeking a Care Partner to support older Australians in the Kinglake West region. You will manage a home-based caseload, conduct assessments, and develop personalised Care Plans to help clients stay secure and connected in their own homes.

The role combines community visits with the flexibility to work autonomously from home, offering full-time permanent employment and opportunities to build meaningful relationships in a supportive team.
