Uniting is seeking a Neighbourhood Care Partner in Broken Hill to lead case management and deliver person-centred home and community care services across NSW and ACT. You will be the central contact for clients, coordinating care planning within available funding to support independent living.
The role requires the ability to travel between Uniting sites and client locations; you will advocate for client choice, safety and quality of life, while partnering with the broader care team.
