Wellways is seeking a Health Linker to help people access health, community and social supports across the Murrumbidgee region. You will work with GPs and local services to create coordinated pathways that improve wellbeing and recovery goals.
The role requires person-centred, trauma-informed practice and solid collaboration with multidisciplinary teams. You will advocate for social inclusion, reduce isolation and promote participation in local networks.
