Job Description

Hammond Care seeks a permanent part time Clinical Partner - Nursing to support clients in the Penrith, Hawkesbury and Lower Blue Mountains region. The role involves 3 days per week, driving between clients' homes to provide tailored home-based nursing care.

You will join a multidisciplinary Home Care Clinical Care & Practice Team, performing home visits, assessing needs, and delivering person-centred care with a focus on keeping people independent at home.
