Monash Health in Clayton is seeking an experienced Grade 3 Social Worker to join the Renal Supportive Care team.
This ongoing part time (0.4 EFT) role provides advanced psychosocial support to patients with advanced CKD and their families, in a multidisciplinary setting across nephrology and palliative care.Based at Monash Medical Centre, you will work autonomously within multidisciplinary teams, contributing to care planning, advocacy, and education for healthcare professionals.
