You'll work closely with clients and their families to develop individualised care plans,



set achievable goals and connect them with services that enhance their wellbeing.Your day may include:Visiting clients in the community referred for help at homeDeveloping and evaluating care plans tailored to individual needsBuilding relationships with families and carersCollaborating with multidisciplinary teams to implement best practice careDocumenting client progress using care management systemsPromoting BaptistCare's services and engaging with prospective clients to support growth in the local communityWhat You'll BringRequired qualifications and experience:Bachelor's Degree or Diploma in Community Services (Service Coordination or Case Management), or equivalent experienceExperience in the aged care sector or demonstrated transferable skills within another community‐based service environmentAbility to work independently in the community and as part of a teamStrong communication, time management, and organisational skillsIntermediate computer skills and proficiency with email and appsValid Australian Driver Licence and reliable vehicle with a minimum Third Party Property Damage InsuranceAbout UsBaptistCare is a leading purpose-led care organisation, guided by our Christian identity and serving people and communities across Australia for more than 110 years.
