The role requires hospital-based experience, solid discharge planning, and psychosocial interventions to support patients and families.Located at The Alfred, it is a fixed-term, full-time role with weekend work once per month.
Applicants should hold a Bachelor Degree in Social Work and have experience with EMR systems, NDIS, aged care and VPTAS processes.
