Amplar Health Home Hospital is expanding its Transition Care Service to Walkerville — providing another welcoming location for public hospital patients who no longer require acute care.
Here, patients can rest and recover in comfort, while helping free up hospital beds for those who are acutely unwell.
This expansion builds on our innovative model of care, enabling patients to move from metropolitan and peri-urban hospital wards into a supportive sub-acute setting designed for recovery and rehabilitation.Start your journey with us as part of the foundational team at our brand‐new site — and be ready to welcome patients when admissions begin.No two days will be the same: you'll care for patients recovering from surgery, illness, or injury, helping them regain strength and confidence, in this sub-acute nursing role.Your opportunityThis opportunity will provide you a chance to exercise your experience as a Registered Nurse,



providing comprehensive care to patients with a broad range of needs in a recent dynamic setting outside of the hospital, in addition to:Delivery high quality patient care across diverse clinical presentationsCoordination and delivery of high-quality patient centred care within a multidisciplinary environmentWork in a professionally simulating, rewarding environment that utilises innovative technologyRespond confidently to clinical changes and other emergency situationsMentor junior staff and contribute to continuous improvementParticipate in ongoing professional development and trainingAbout youYou'll enjoy a balanced 7-day rotating roster (mornings, afternoon and nights) offering variety, flexibility, and work-life balance, within a supportive team environment.Experience as a Registered NurseHold current First Aid and CPR certificatesHold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
If successful, willing and able to provide proof of up-to-date vaccinations.Nurses with acute care experience are encouraged to apply.
